Luma currently provides telehealth medical services only to eligible patients physically located in Florida or New York at the time of care. Care is provided by Shirel Toren, FNP-C, a licensed Nurse Practitioner. We do not provide medical care to patients located outside Florida or New York.

• You must be 18 years of age or older.
• You must be physically located in Florida or New York at the time telehealth medical services are provided.
• You must be able to provide accurate identification and medical history and be clinically appropriate for telehealth care.

• Provide complete, accurate, and current information about your health history, medications, allergies, and symptoms.
• Disclose your current physical location at the time of each visit.
• Keep your account credentials confidential and your contact details current.
• Use the platform lawfully and follow your provider's instructions.

Telehealth cannot replace an in-person physical examination and is not appropriate for every condition. Your provider may refer you for in-person evaluation, urgent care, emergency care, laboratory testing, or additional workup when clinically appropriate.

A consultation does not guarantee a diagnosis, a prescription, a specific medication, or eligibility for any program. Prescription medications are provided only when clinically appropriate following a medical evaluation. Treatment options vary based on medical history, clinical eligibility, availability, and applicable state and federal requirements. No specific outcome or result is promised.

Prescriptions, when issued, are transmitted to a licensed pharmacy. Some treatments may involve compounded medications, which are not FDA-approved and are not reviewed by the FDA for safety, effectiveness, or quality. Pharmacy availability, sourcing, and shipping timelines are determined by the dispensing pharmacy. Controlled substances and certain other medications may not be prescribed by telehealth.

• Consultation and visit fees are charged in advance and pay for provider evaluation time, whether or not a prescription is issued.
• Medication costs, where applicable, are stated on the relevant service page. Medication is not automatically included where eligibility must first be medically determined.
• Laboratory testing is billed separately at the amount shown on the service page.
• Shipping charges, where applicable, are shown at checkout before you complete your order.
• Where a treatment is billed on a recurring basis, the amount and frequency are shown before checkout and you may cancel before the next scheduled charge. Luma does not currently offer memberships.
• Prices are subject to change; the price shown at checkout applies to your order.

By providing your email address and phone number, you consent to receive service-related messages about your care, appointments, and orders. Marketing messages are sent only with your consent and you may opt out at any time. Secure messaging is not monitored 24/7 and must not be used for emergencies.
